Payroll Deduction Agreement

I am currently employed at Massachusetts General Brigham and am a regular status employee with 20 or more standard hours and am eligible to receive benefits from my employer.   

In order to receive a discounted membership rate at The Clubs at Charles River Park, I authorize my employer to deduct from my pay once a month/ week the amount indicated above.

I may authorize additional deductions from my pay for individual services rendered, but no discount will apply to such services.  

I must cancel my club membership, if my employment is terminated for any reason, or that my standard hours are reduced to fewer than 20. My discounted membership rate will continue only through the end of the last month that I am paid.  

Membership is month to month; however, I understand that if I cancel my club membership, I may be charged a rejoin fee until one year from the date of cancellation.*  

Monthly and weekly cancellations are effective at the end of the month. To cancel monthly or weekly deductions, signed written notice must be received at least 2 weeks in advance of last month membership is desired.        

*Consideration will be given for approved employment leave, medical reasons and house staff rotations.  Documentation may be required. Employees may not freeze this membership.
